Abstract

85-year-old woman with multiple comorbidities: Chronic obstructive pulmonary disease and heart failure. Hospitalization one month ago for pulmonary thromboembolism managed with rivaroxaban 20 mg/day; mixed delirium and dementia treated with risperidone, initially 0.5 mg/day and then 1 mg/day upon discharge. Hospitalary readmission by urinary septic shock related to infection by Klebsiella Pneumoniae KPC, treated with ceftazidime/avibactam 2.5 gr every 8 hours Blood count (BC) on admission with 770 leukocytes/mm3, hemoglobin 6.3 gr/dl, hematocrit 18.9% and platelets 61,000/mm3. Risperidone was suspended and haloperidol was started, two units of red blood cells were transfused, showing improvement on fourth day, 1.840 leukocytes/mm3, 740 neutrophils/mm3, hemoglobin 9.8 gr/dl, hematocrit 29.3% and platelets 227,000/mm3. Causality of adverse drug reaction evaluated with the World Health Organization algorithm was classified as “possible”.
