The most common causes of neuropathy are diabetes mellitus, alcoholism, amyloidosis. The most common neuropathy in systemic amyloidosis is a small fiber axonal polyneuropathy . The first case is that of a man with Familial Amyloid Polyneuropathy (FAP). He stated with distal burning pain of sudden installation, history of orthostatic hypotension, erectile dysfunction refractory to treatment and gastrointestinal irregularities. The second case is that of a woman with systemic amyloidosis presenting with neuropathic pain in the lower extremities, diarrhea and fever for four weeks with a neurological examination and a normal EMG
