Abstract

Introduction: In spite of pharmacological breakthroughs in coronary heart disease treatment, still there are difficulties in achieving therapeutic targets in clinical practice. to bridge the gap, we proposed a secondary prevention program at Hospital Italiano de Buenos aires. the objective of this paper is to describe how this program will be implemented and evaluated. Materials and Methods: Quasi-experimental study with historic control: intervened prospective cohort compared to a retrospective control cohort. Follow-up period will be 3-year long. Eligible patients will be between 18 and 79 years of age, affiliated to Hospital Italiano’s Health Plan, admitted to our coronary unit for an acute coronary syndrome, with or without St segment elevation. During the program, the health-care team will work with patients applying cognitive-behavior techniques to promote lifestyle changes. they will stimulate patient’s self-care and will coordinate their attention to assure the continuum of care. according to patient’s needs, the interventions could be related to: anti-tobacco treatment, physical activity, nutritional education, overweight/obesity treatment, surveillance of modifiable risk-factors control, and pharmacological treatment adherence. Program effectiveness will be evaluated through a composite indicator of health-care quality. conclusions: considering that a great number of patients with coronary heart disease cannot achieve therapeutic goals, it is necessary to develop a program based on a patient-centered approach through a multidisciplinary team. long-term effectiveness of this program must be evaluated
